For practitioners

Send standard questionnaires and add up their scores

A tool to send standard questionnaires and add up their scores by their published keys; the clinical judgement is yours.

How it works

  1. In your cabinet you choose a questionnaire for a client and make a link. You send it yourself, by e-mail or message.
  2. Your client answers on their own device. Your answers stay on this page until you send the link at the end back to them yourself. They never see a score.
  3. They send you the link the page gives them. You paste it into your cabinet, and the answers are added up by the questionnaire's published key, beside every item answer.
  4. Nobody watches answers as they arrive: you see a return only when your client sends it and you open it. Following up what a client answers, including item 9 of the PHQ-9, is yours.

Where the records are

Your clients, your questionnaire links and every return are kept in this browser only, on this device. MyTestAtlas never receives them, so it cannot restore them.

Clearing this site's data, or a browser that clears it for you, deletes them. Safari and every browser on iPhone and iPad delete what a site has stored after 7 days without a visit, unless the site is added to the Home Screen.

Export a file regularly and keep it where you keep client records. The export is encrypted with a passphrase you choose.

Each link carries a check that ties its return to the request made on this device. It does not stop someone who saw the request link from answering it. A link opened in a messaging app's browser, or kept in synced browser history, stays in that app or history.

The questionnaires

Each is reproduced word for word from its authors’ published form, with their attribution, and added up the way they publish. The cabinet shows the authors’ own reference text beside a total and adds no reading of its own.

Practitioner tool beta

Send standard questionnaires to clients and keep the answers on your own device. Planned price: $9 a month.

Free during the beta. Nothing is charged unless you choose to pay. We'll tell you in your cabinet at least 30 days before billing starts. If you don't, nothing on our side changes: your records are on your device, and you can still export them.

What we store

Your Google sign-in and session, that you joined the beta and that you confirmed the practitioner statement. Nothing about your clients: no label, no link, no answer and no total reaches us. The pages are served by our hosting provider, which sees the address of the page and your browser’s request, never the part of a link after the #. The full detail is in section 2.12 of the privacy policy.

Questions people ask

Does my client need an account?

No. They open your link, answer and send the link back. Only you sign in.

Can my client see their score?

No. The client's page never adds anything up. You go through the answers with them.

What happens if a client answers item 9 above 0?

Your client sees support lines at once, on the page. You see "Item 9 answer" with the number when you add the return; nobody is told before you open it, so follow-up is yours.

What if I clear my browser or change computer?

Your records are only in this browser. Export a file regularly; you can load it in another browser to carry on.
