Is this the standard self-esteem questionnaire?
No. The best-known one is Morris Rosenberg's scale, and the permission Dr. Florence Rosenberg gave, published by the University of Maryland, covers educational and professional research, with credit to Dr. Rosenberg. A free public web page is neither of those things, so every question here was written from scratch.
Can self-esteem change?
Public health services describe self-esteem as the opinion you have of yourself and as something that can be worked on with attention and daily practice. That is why this page names the closest pattern in this sitting rather than a level you are stuck at.
Is low self-esteem a mental health condition?
No, it is not a diagnosis. Health services do say it can sit alongside low mood, and Victoria's Better Health Channel advises seeing a doctor if you are having trouble improving your self-esteem or if it is causing problems such as depression.
